Use these order forms for Standard and Custom equipment listed on the Product page, and Custom Services

There are 3 forms:

Form 1 (Standard Equipment Ordering Form) must be printed and sent. This is the only form you will need if you require only standard equipment from the product list.
Form 2 (Custom Vertical/Horizontal Electrophoresis units, UV-Transparent Trays and Glass Plates) contains diagrams and require your dimensional specifications.
Form 3 (Custom Combs and Radiation Shielded Boxes) contains diagrams and require your dimensional specifications.
Forms 2 and 3 must be used for custom equipment.

Print the form(s), and fill in the appropriate fields as thoroughly as possible.
